If there is any question or concern about the safety and treatment of a patient, we strongly encourage to do any of the following:

1. Notify the charge nurse or a social services clinician or care manager

2. Speak with the attending physician

3. Speak with the Patient Ombudsman

4. Speak with the Director of Performance Improvement*
